For Old Pupils (Returning Students):
- Latest Report Card (for Grade School)
- Developmental Checklist (for Preschool)
For New Pupils / Transferees:
- Latest Report Card or Checklist
- Copy of PSA Birth
- Certificate of Good Moral Character
- Passed Admission Assessment administered by PHKCI
- Permanent Record (Form 137) to be requested by PHKCI
- Permit to Study from the Department of Education (for foreign pupils)
- Clearance from the Business Office (if Report Card is not yet available)
